Changes to the BPA Tandem Fitness form

As of this week (April 11th) the govering body the British Parachute Association has introduced two forms to replace the original tandem fitness form (Form 115).

The 40+ age limit has been removed for mandatory medicals for tandem jumpers.
Ability to take part in a tandem jump is now solely based on medical history.

If you have already booked your jump and have the original declaration of fitness form signed and stamped by your doctor this will remain valid up until it’s expiry date.  

If you are thinking of jumping or have booked and have not already had this form signed, please discard the original and use the new forms discussed below.

Form 115a: BPA Tandem Student Parachutist Medical Information & Declaration

  • If you are able to sign this, and therefore self-declare, please complete and bring this form with you on your tandem day.
  • Please be honest.  If you are unable with certainty to sign the form do seek medical advice.
  • If you are unable to sign the new form based upon the statements made you will need to print the second form 115b and obtain suitable medical advice and bring the signed form with you on your tandem day.

Form 115b: Tandem Student Parachutists Doctors Medical

  • As before, failure to produce the relevant form means you will not be able to jump
  • As before a letter form the doctor, notes or any other type of medical cannot be accepted. It must be the official signed BPA form 115a or 115b only.  Forms should be brought along on the day, please DO NOT send them in advance and do not hand them over to charity or group representatives.

Again
The old tandem form 115 is still acceptable and valid until their stated expiry, so if you have already had this form signed by your doctor you do not need to have them replaced.
